Course Overview 

Effective investigations are a key activity of any Safety Assurance program. Safety occurrences and incidents, when properly investigated and analysed, have the potential to facilitate proactive safety improvements within an organisation. These free lessons may be used to prevent the occurrence of more serious incidents and accidents.

This Safety Investigators course has been especially designed for those involved with the investigation and analysis of occurrences and events including incidents, near misses and quality problems. This includes management at all levels and safety assurance personnel.

The course includes a mix of theory and practical, with emphasis on a systems-based approach to investigation using contemporary safety analysis. The course also includes a full safety investigation, which allows the participants to put the knowledge and skills learned into practice, in a realistic workplace-based scenario. During this exercise participants will explore and utilise a number of analysis tools, including Incident Cause Analysis Method (ICAM) and other tools. 

Overall, participants will develop both the knowledge and the practical skills needed to investigate and analyse operational incidents, with a view to developing improvements within their organisation.

The Safety Investigators course explores and utilises a number of investigative tools for data collection and analysis. The analysis tools used and discussed include:

  • Incident Cause Analysis Method (ICAM) – (Principal methodology)
  • 5 Why’s
  • Ishikawa (Fishbone) Diagram
  • Failure Modes Effects Analysis
  • Fault Tree Analysis
  • Tap Root Analysis
  • Human Factors Analysis & Classification System (HFACS)
  • Functional Resonance Analysis Methods
  • STAMP/CAST
  • Accimap

Upon successful completion of this course, including all assessments, participants will receive a statement of attainment including:

  • BSBWHS515 Lead initial response to and investigate WHS incidents

Participants will also receive a Southpac Aerospace endorsed Safety Investigators Course certificate.
